Please make sure that you have all of your paperwork in before school begins so you can practice with us when you get back from the summer break. Check below for the paperwork packet needed for participation.
Monarch High School Athletic Participation Packet
Includes: Athletic Information Sheet, Sportsmanship Policy, Physical Form, Consent and Release Form (Needs
to be notarized), Broward Health Authorization for Release and Consent for Treatment forms.
Please fill out all forms completely. Note: the student's birth certificate only needs to be on file once throughout
high school, but all the other paperwork needs to be filled out yearly. You may fill out some of the forms online, but you will need to print them for signatures and for the phyiscal's completion by a doctor.
